100% especially if you can't speak the language or don't know the lay of the land. They can smell a foreigner once the plane hits the tarmac. Everything from how you walk to your attire, they will know immediately.

And on top of that, locals aren't really hanging out and about in Sosua like that (not counting locals that work there or vendors/beggers).

Will they treat you like a rap star celebrity and worship the ground you walk on? (which I think is the REAL question you are trying to ask)? No. Those days are long gone since many black men are hitting Sosua nowadays. The chicas have become too familiar with you and don’t look at you twice.

For the most part, yes. In such instances you don't even have to speak. Physically, many American black men have a larger body frame and carry their weight differently than Dominicanos.

Dead on. Regarding the gait thing, many of us suffer from sports related injuries that make our later years just a bit difficult. They don’t play football here so in general Dominicans aren’t subject to that type of life long disability.

Honestly if the light skin to a lighter shade of brown and have short hair, it can confuse some of the Dominicans and Haitians from my personal experiences. They going based off skin complexion initially then they listening to see if it’s a local or foreigner. Just be in shape, recent haircut, wear fairly new clothes and you won’t be an outlier. So maybe 15-20% of the guys. Guys don’t have to be mixed because it’s the Caribbean so it’s a paper bag test first, then they looking at clothes then they listening To the guys speak. Some well off Dominicans wearing cornrows, and box braids with jewelry and have money to rent cars/motos/four wheelers and have tattoos
Cheaper ways to do things, doesn’t indicate quality, or lack of

My two cents. 

You can be identified as a foreigner by chicas when :  you are seen regularly spending money, at tourist establishments.  Or paying tourist prices.   

When chicas see a guy walking out feeling refreshed after paying 600 to 1000 pesos for a massage, he's not a Dominican.   

When chicas see a guy staying at a hotel in Sosua paying tourist prices, or renting an apartment over $300 USD a month he's most likely a foreigner.

If chicas see a man trying to buy stuff using a credit or debit card, or foreign currency other than DOP......he's likely a foreigner.   

If the chicas see a guy take his phone out to take pictures or video of anything, he's likely to be a foreigner.   
Where you stand on the issue depends on where you sit.  The words enough, some, many , to whom it applies to, does not mean ALL of any people, places or things.  If you cant, dont or wont learn from friends, family or your peers, dont worry the locals will teach you, and you will learn the hard way.
